Please indicate the domain of practice for which you are seeking a restricted registration. Please ensure that you have the level of experience required for the type of service and type of location you will be providing service, and that the level of supervision will be required.

Critical Care
Teaching-Tertiary 3 12 months of anesthesia at PGY2 or higher. 2 months of ICU at PGY2 or higher. Anytime after completion of 2 months of critical care rotations to end of residency >=3/5** Moderate
Teaching-Community 3 12 months of anesthesia at PGY2 or higher. 2 months of ICU at PGY2 or higher Anytime after completion of 2 months of critical care rotations to end of residency >=3/5** Moderate
Non-Teaching-Tertiary 3 12 months of anesthesia at PGY2 or higher. 2 months of ICU at PGY2 or higher Anytime after completion of 2 months of critical care rotations to end of residency >=3/5** Moderate
Non-Teaching-Community 3 12 months of anesthesia at PGY2 or higher. 2 months of ICU at PGY2 or higher Anytime after completion of 2 months of critical care rotations to end of residency >=3/5** Moderate
** Must meet expectations in all areas. Cannot have any weaknesses in evaluation scores or narratives.
